Hypoechoic mass in the parotid gland and an enlarged lymphe node

77 year old female, Palpable painless mass

There is a hypoechoic mass in the right parotid gland and an enlarged lymphe node next to the parotid gland.

The mass has an irregular contour and contains some calcifications

Image of the mass with e-flow. There is a very irregular vascularity.

Conclusion

Although many of the salivary gland tumors are benign, in this case the irregular contour, the irregular vascularity and the presence of an enlarged lymphe node, we found the mass very suspicious for a malignant tumor with a lymphe node metastasis. We performed an ultrasound guided biopsy of the mass and the enlarged lymphe node. The conclusion of the pathologist was: Malignant parotid gland tumor acinic type with lymphe node metastasis.
