Retro-aortic left renal vein (RLRV)

Posterior NutCracker Syndrome

Clinical information

Patient was presented with a nagging pain for one month in the left loin.
No signs of dysuria, polyuria or hematuria.

Brief description

Ultrasound showed a normal appereance of both kidneys.
No signs for hydronephrosis or the presence of kidney stones.
By chance a retro-aortic vessel appeared joining the inferior vena cava in the orthotopic position.
On an abdominal CT scan the vessel was identified as a retro-aortic left renal vein ( RLRV) without the risk of compression.
RLRV is a rare condition with a prevalence of 1%-2%

Conclusion

RLRV is usually asymtomatic, it may cause loin pain and hematuria.
These symptoms are secondary to compression of the left renal vein between the abdominal aorta and vertebrae. This condition is known as posterior nutcracker syndrome ( PNCS).
