Conclusion

The only structure between the gallbladder and the pancreas is the duodenum. The air within the mass is suggestive for a part of the digestive tract. Endoscopy showed a large ulcerating duodenal mass that biopsy proved to be an adenocarcinoma of the duodenum. The CT scan and US showed no distant metastases.
The patient was referred to an oncological center to see if surgical removal (Whipple procedure)is still an option
